ILLY are pleased to announce the start of our new partnership working with Safer Portsmouth Partnership to deliver LINKS CarePath across their new Recovery Orientated Integrated System for substance misuse.
The objective of the new treatment model is to create a consistent client-centred recovery plan, ensuring continuity of care and service personalisation. The visibility of information across the Partnership will be improved and the business processes should benefit as a result.
The multi-agency working will be delivered by the following:
- Society of St James (psychosocial interventions)
- Solent (prescribing)
- Portsmouth Counselling Service (one to one psychosocial)
- Portsmouth Alcohol Interventions Team (alcohol services)
- Cranstoun – SWITCH (young people’s service)
- Portsmouth City Council (The Referral & Assessment Recovery Hub including DIP)
- Community Rehab programme (starts 1st Sept)
ILLY’s proposed a solution to implementing LINKS CarePath across the Hub and Spoke Model to improve care pathways. This will increase focus on recovery integration, maximise the communication channels and enhance continuity of care among services. The flexibility LINKS CarePath offers along with ILLY’s comprehensive implementation process ensures the system will be tailored to Portsmouth’s requirements.
The ILLY and PCC project teams have been working closely together to create a plan and agree on an approach to deliver the LINKS CarePath system for 1st July 2013.
